Zusammenfassung

Die peptische Ulcuserkrankung ist eine häufige Erkrankung. Die Prävalenz wird im Mittel für das Ulcus ventriculi mit 0,86% und für das Ulcus duodeni mit 7,7% angegeben [33]. Die Incidenzrate liegt für das Ulcus ventriculi bei 0,3/1000 pro Jahr und für das Ulcus duodeni bei 2,89/1000 pro Jahr [33]. Diese bei Ärzten in Massachusetts erhobenen Incidenzraten stimmen mit den Befunden aus der Grafschaft York in England gut überein [39]. Interessant ist in diesem Zusammenhang, daß in den letzten Jahrzehnten auf beiden Seiten des Atlantiks eine Abnahme der Häufigkeit der Ulcuserkrankungen festgestellt wurde, deren Ursache letztlich ungeklärt ist [30, 33, 39,48]. Befunde aus der Schweiz [51] belegen bei jungen, männlichen Gastarbeitern eine deutliche Häufung an Ulcera duodeni im Vergleich zur Schweizer Bevölkerung. Diese Zunahme der Ulcuserkrankung wird mit Assimilationsschwierigkeiten in einer sozial und kulturell unterschiedlichen Umwelt erklärt. In Südafrika dagegen wurde in den Städten keine Häufung der Ulcuskrankheit bei den niedrigsten sozialen Schichten beobachtet, sondern erst bei der schwarzen Bevölkerungsgruppe, die in besser bezahlten Berufsgruppen tätig ist [43]. Diese Unterschiede in den epidemiologischen Befunden machen weitergehende Untersuchungen zur Beantwortung der Frage erforderlich, warum nur bestimmte Formen von „Streß“ möglicherweise die Ausbildung peptischer Läsionen begünstigen.
