Zusammenfassung
Bei der chronischen arteriellen Hypertonie wird nach ätiologischen Gesichtspunkten unterschieden zwischen einer primären essentiellen und einer sekundären Hypertonie (Tabelle 1). Zu den sekundären Hypertonien gehören die renal-parenchymatösen Hypertonien bei beidseitigen und fakultativ einseitigen Nierenerkrankungen, die renovaskulären Hypertonien, die endokrinen Hypertonien bei Phäochromozytom, Conn- und Cushing-Syndrom und die Hypertonie bei Isthmusstenose der Aorta. Bei den sekundären Hypertonieformen kennen wir mit Ausnahme des Phäochromozytoms im Grunde genommen auch nur zusätzliche Krankheiten oder Funktionsstörungen, von denen wir annehmen, daß sie die Blutdrucksteigerung ausreichend erklären. Dieser bekannte ätiologische Zusammenhang der sekundären Hypertonieformen mit einer bestimmten Grundkrankheit darf jedoch nicht zu der Schlußfolgerung verleiten, daß hiermit auch die Pathogenese der arteriellen Hypertonie bekannt ist. Bei der essentiellen Hypertonie, die mit einem Anteil von 80–90% immer noch die häufigste Hochdruckform darstellt, gestehen wir uns schon mit der Bezeichnung ein, daß wir nichts oder nicht viel über ihre eigentliche Genese wissen.
