Zusammenfassung
Die Frage, ob eine erhöhte plasmatische Gerinnbarkeit imstande ist, eine Thrombosebereitschaft hervorzurufen, konnte durch lange Zeit nicht sicher beantwortet werden. Im Zusammenhang damit bestand das Problem, ob der Begriff einer „Hyperkoagulämie” überhaupt als Änderung der plasmatischen Gerinnung definiert werden kann. Eine Konzentrationssteigerung inaktiver plasmatischer Gerinnungsfaktoren über den Normbereich wurde zwar häufig beobachtet, doch führte sie im allgemeinen nicht zu einer erhöhten Thrombosefrequenz. Allerdings wurden dabei einige Ausnahmen beschrieben. So konnte bei einer Familie mit erhöhter Faktor-V-Aktivität eine beträchtliche Thromboseneigung bereits im Kindesalter festgestellt werden [13], ferner wurden Fälle mit Thromboseneigung beschrieben, bei denen als einziger abnormer Befund eine hohe Faktor-VIII-Konzentration gefunden wurde [29]. Es ist allerdings nicht bekannt, ob es sich bei den erwähnten Fällen um normale Proteinmoleküle oder um solche mit einem modifizierten Aufbau handelte. Dieses Problem wurde besonders deutlich durch die Beschreibung einer Familie mit hoher Thrombosefrequenz, bei der die Fibrinogenkonzentration zwar normal war, das Fibrinogen aber eine beschleunigte Reaktion mit Thrombin aufwies. Dieser Befund ließ auf ein verändertes Fibrinogenmolekül schließen [11].
