Hemodynamic Response to External Counterpulsation

Abstract

External counterpulsation (ECP) is a noninvasive method of mechanical cardiac assistance. The legs of the patient lie in two rigid body sleeves that themselves are contained in water-filled sleeves. An intermittent diastolic pressure, triggered by the ECG, is applied to these sleeves, which then exert a uniform pressure on the legs and the blood vessels. This diastolic pressure wave produces additional circulatory impulses; a secondary aim is the reduction of systolic pressure and thus a reduction of left ventricular ejection resistance. Optimal adjustment of the pressure wave is controlled by means of a finger Plethysmograph; when correctly adjusted the pressure wave leads to high diastolic augmentation (Fig. 9.1)
