Fascicular Block Patterns — Observations on Differential Diagnosis

Abstract

Fascicular block patterns merit clinical interest because they are the most frequent precursors of acquired complete atrioventricular block [6, 16, 22, 24, 26, 28]. In the present paper we lay stress on differential diagnostic aspects. The behavior of the intrinsicoid deflection and the presence of mirror image effects may be helpful for diagnosis whereas interferences between fascicular blocks and infarction patterns may render a correct diagnosis more difficult. The anatomical variations of the intraventricular conduction system, as well as the age, the physical constitution, and the clinical diagnosis of a patient are always to be considered. The difficulties in recognition of the left anterior fascicular block are shown on the basis of 100 consecutive ECGs with left axis deviation.
