Akutes Nierenversagen (ANV)

Summary

Acute renal failure is defined as a multifactorial Syndrom that occurs — depending of definition and selection of patients — in 2%–25% of all ICU patients. Most frequent in surgery (~70%), ARF is caused by septic complications. Renal replacement therapy is managed today by dialysis, hemofiltration or the combination of both. Lethality of ARF ranges from 50% to 80%. High lethality rates are not related to ARF itself, but to uncontrolled septic MOF. The consequences for surgical ICU patients are:

• First of all, protective measures are mandatory.

• If ARF occurs, septic complications have to be considered.

• Renal replacement therapy has to be integrated into the management of septic MOF.

Zusammenfassung

Das ANV ist ein multifaktoriell verursachtes Syndrom, das — je nach Definition und Krankengut — bei 2% bis 25% aller Intensivpatienten auftritt. Häufigste Ursache in der Chirurgie ist mit ca. 70% die septische Komplikation. Als Nierenersatztherapie kommen heute Dialyse, Haemofiltration und Kombinationen beider Verfahren zur Anwendung. Die Letalität des ANV liegt zwischen 50% und 80%. Die hohe Letalität geht nicht zu Lasten des ANV per se, sondern zu Lasten des nicht beherrschten septischen MOV. Die Konsequenzen für den chirurgischen Intensivpatienten sind:

• Im Vordergrund müssen protektive Maßnahmen stehen.

• Tritt ein ANV auf, muß immer und zuerst an die septische Komplikation gedacht werden.

• Die Nierenersatztherapie muß in das Gesamtbehandlungskonzept des septischen MOV integriert werden.
