Perioperative Antibiotikaprophylaxe in der operativen Gynäkologie

Summary

Cephalosporins are the agents of choice for antimicrobial prophylaxis in gynecologic and obstetric surgery. They are administered as a preoperative Single dose. In patients allergic to penicillin, doxycycline, Clindamycin, or metronidazole are used instead. In cesarean section, the antibiotic is given after clamping of the umbilical cord. The effectiveness of antibiotic prophylaxis has been proven in hysterectomy, oncologic surgery, breast surgery, and cesarean section. It is also recommended for hysterosalpingography, infertility surgery, insertion of an intrauterine device, cervical cerclage, and other procedures.

Zusammenfassung

Zur modernen Antibiotikaprophylaxe in Gynäkologie und Geburtshilfe werden vor allem Cephalosporine — bei Penicillinallergie Doxycyclin, Clindamycin oder Metronidazol — als präoperative Einmaldosis verwendet. Bei der abdominalen Schnittentbindung wird das Antibiotikum erst nach d em Abnabeln des Kindes verabreicht. Die Wirksamkeit der Antibiotikaprophylaxe ist bei Hysterektomien, onkologischen Operationen, Brustoperationen und der abdominalen Schnittentbindung nachgewiesen. Aber auch bei Sterilitätsoperationen an den Tuben, bei der Hysterosalpingographie und der Einlage von Intrauterinpessaren wird eine Prophylaxe mit Doxycyclin empfohlen.
