Summary

The introduction of the nasal airway access, as an alternative to the endotracheal or the tracheostomal tube, has extended the application of inspiratory positive pressure ventilation to patients in a chronic state of disease and outside the intensive care unit. Nasal mechanical ventilation is noninvasive and can be performed by patients whenever the necessity arises.

Patients in a state of chronic ventilatory failure due to muscular and chest wall disorders improve constantly during intermittent nasal ventilation. The overall improvement in patients with chronic obstructive pulmonary disease (COPD) can not be predicted, but noninvasive ventilation is a realistic alternative in selected patients when conventional therapy has failed.
