Zusammenfassung
Der Sepsisforschung kommt in der Chirurgie und der begleitenden chirurgischen Forschung seit je her ein hoher Stellenwert zu. Der Operateur empfindet es oft als persönliche Niederlage, wenn sein Patient postoperativ „septisch“ wird, wenn eine Focussuche erfolglos bleibt oder eine chirurgische Focussanierung nicht vollständig gelingt, eine gezielte Antibiotikatherapie nicht den erhofften Erfolg hat und auch die Maximierung der intensivtherapeutischen Maßnahmen letztlich erfolglos bleibt. Obwohl die Entwicklung moderner Breitbandantibiotika für die Therapie von Infektionskrankheiten große Fortschritte erbracht hat, so müssen doch die Erfolge für die Sepsistherapie als eher bescheiden angesehen werden. Dies läßt darauf schließen, daß Sepsis und septischer Schock nicht nur das klinische Symptom einer mikrobiellen Entgleisung sind, sondern daß hierfür nach heutigen Vorstellungen die außer Kontrolle geratenen, sich selbst verstärkenden Reaktionen der körpereigenen Abwehrmechanismen mit ihren Mediatoren verantwortlich sind, in deren zeitlichem Verlauf einzelne Organe und Organsysteme insuffizient werden, ganz versagen und schließlich für den Tod des Patienten verantwortlich sind.
