Zusammenfassung
Aus der Literatur ergibt sich, daß die meisten stationär behandelten unfallchirurgischen Patienten, insbesondere mit Verletzungen der Wirbelsäule, des Beckens und der unteren Extremitäten, zu den Hochrisikopatienten gezählt werden müssen, die einer individuell angepaßten medikamentösen Thromboembolieprophylaxe, insbesondere einer höher dosierten Heparinprophylaxe als sonst in der Chirurgie üblich bedürfen. Gerade in der Behandlung mit niedermolekularen Heparinen und Heparinoiden scheint hier noch ein zu erforschendes Potential zur Verbesserung der Thromboembolieprophylaxe zu liegen. Für den ausschließlich ambulanten Bereich sind in jedem Falle individuelle Risikofaktoren der Patienten zu beachten, und im Zweifelsfall ist eine medikamentöse Thromboembolieprophylaxe einzuleiten, da die hier inzwischen bekannt gewordenen Thromboseraten — insbesondere bei konservativer Behandlung von Frakturen der unteren Extremitäten — den Bereich des mittleren Risikos erreichen.
