Zusammenfassung
In einer retrospektiven Studie wurden 76 Augen von 60 Patienten untersucht, bei denen zwischen 1989 und 1995 eine Katarakt-OP mit Implantation einer Plan- oder Minuslinse durchgeführt worden war. Die Bulbuslängen lagen zwischen 26,6 und 36,9 mm, im Durchschnitt bei 31,8 mm. Die Nachbeobachtungszeit betrug bis zu 6 Jahre und lag im Mittel bei 28 Monaten. Postoperative Komplikationen traten in 6,6% der Fälle auf. Eine Pseudophakieamotio fand sich nur bei 1 Patienten. In 30 Fällen wurde eine Nd: Yag-Lagerkapsulotomie notwendig. Die mittlere Abweichung von der errechneten Zielrefraktion betrug ± o bis -0,25 dpt (Standardabweichung ± 1,25). Die Refraktion blieb über den gesamten Beobachtungszeitraum hinweg stabil (P > 0,05). Zu einer Visusverbesserung kam es in 96,8% der Fälle.
Aufgrund der geringen Komplikationsrate erscheint eine zurückhaltende Einstellung zur Linsenentfernung mit IOL-Implantation als optische Rehabilitation bei exzessiver Myopie nicht angezeigt. Dieses operative Vorgehen könnte daher insbesondere wegen der guten Berechenbarkeit und Stabilität der postoperativen Refraktion auch als „clear lens extraction“ eine gleichwertige Alternative zur refraktiven Hornhautchirurgie bei hochmyopen Augen darstellen.
Summary
In a retrospective study, 76 eyes of 60 patients who had undergone cataract surgery and implantation of a plane or minus IOL between 1989 and 1995 were examined. The bulb lengths were 26.6-36.9 mm, at an average of 31.8 mm. The follow-up period was up to 6 years, at a mean of 28 months. Postoperative complications could be observed in 6,6% of the cases. Pseudophakic detachment was only found in one patient. In 30 cases a Nd: YAG laser capsulotomy had to be carried out. The mean deviation from the calculated target refraction was ± o to -0.25 D (S.D. ± 1.25). The fraction remained stable over the entire follow-up period (P > 0.05). Vision could be improved in 96.8% of the patients.
Due to the low complication rate, a hesitant attitude towards the application of lens removal and IOL implantation in the optical rehabilitation of patients with excessive myopia does not appear justified. Especially based on the good calculability and stability of the postoperative refraction, this surgical approach could also be an equivalent alternative to refractive surgery in high myopic eyes when applied as clear lens extraction.
