Zusammenfassung
Seit dem vorigen Jahrhundert, als Nicoladoni [22] eine Fistel zur Entleerung eines Zenker-Divertikels anlegte, befaßt sich die Chirurgie mit der Therapie pharyngoösophagealer Funktionsstörungen. Die erste erfolgreiche Divertikelresektion wurde von Wheeler 1885 beschrieben [28]. Häufige Anastomosenleckagen trugen allerdings dazu bei, einem komplizierten Zweiphasenoperationsverfahren den Vorzug zu geben, mit dem eine kontrollierbarere Speichelfistel geschaffen wurde [14], bis sich schließlich in den 50er Jahren bei der chirurgischen Therapie des Zenker-Divertikels die einzeitige Technik der primären Resektion mit Verschluß durchsetzte [7]. Aubin hatte zwar 1936 [2] eine krikopharyngeale Myotomie zur Resektion des Divertikels erstmals durchgeführt, aber auch dieses Verfahren mußte nach wenigen Jahrzehnten der Erkenntnis weichen, daß die Korrektur der durch den oberen ösophagealen Sphinkter verursachten funktionellen Obstruktion ein für das chirurgische Vorgehen bedeutender Faktor ist [4].
