Clinical implementation of intensity modulated radiotherapy for customised compensation using dynamic multileaf collimation

The Use of Computers in Radiation Therapy

Abstract

The technique of dynamic multileaf collimation (MLC) to deliver intensity modulated radiation therapy (IMRT) has been implemented clinically at our centre since April 1999. Here we describe the planning, delivery and verification methods used to ensure accurate and reliable IMRT treatments. Our initial aim is to provide customised compensation for patient shape in four-field conformal bladder treatments. The clinical intent is to improve dose homogeneity within the planning target volume (PTV) and correct for dose fall-off in the margins of the PTV due to loss of scatter at the beam edge. This is a straightforward adaptation of our current static MLC bladder technique, and acts as a first step towards the introduction of more complex IMRT techniques into the clinic with the potential for shaping dose distributions in the transverse plane [1,2].
