Zusammenfassung
Die Stichinzision des vorgewölbten Hymens bei Hymenalatresie ist als Sofortmaßnahme sicherlich eindrucksvoll hilfreich. Da es aber häufig sekundär wieder zu einem Verschluß des Hymens kommt, wird üblicherweise das folgende Vorgehen empfohlen:
Der Hymenalrand wird an mehreren Stellen bis zur Basis inzidiert und die Inzisionsstellen werden mit einem dünnen, resorbierbaren Faden (Stärke 5/0) an der Basis zur Vereinigung der Scheidenhaut mit der Introitushaut umstochen. Zusätzlich erfolgt eine digitale Dehnung des Introitus. Multiple radiäre Inzisionen des Hymens haben sich als günstiger erwiesen als die zirkuläre Resektion, da die hierbei entstehende zirkuläre Narbe häufig sekundär Beschwerden bereitet.
