Abstract

The majority of patients diagnosed with NIDDM or type 2 diabetes have been obese for many years and are obese at the time of diagnosis. Moreover, quite commonly, weight gain continues throughout the years of living with the disease and, unquestionably, contributes through different pathogenetic mechanisms to the development of the numerous co-morbid conditions which add to the complexity of the management, health care costs, and worsening of disease outcomes. In contrast, weight loss is often associated with significant improvement of the metabolic control and, on occasion, even leads to a complete reversal of the diabetic state. This review will summarize some of the mechanisms involved in the development of type 2 diabetes in obese and overweight individuals. A broad scheme highlighting the pathophysiology of this process is shown in Fig. 1.
