Zusammenfassung
Aus der Kenntnis der Organentwicklung der Nieren und der ableitenden Harnwege ist die Berchreibung angeborener Fehlbildungen abzuleiten. Erst die wesentlichen Schritte der frühen Organbildung erlauben es, die einzelnen Malformationen einzuordnen und ihre jeweilige Organfunktion schon während der intrauterinen Phase zu interpretieren (Armada Maresca et al. 1997). Die wichtigsten Malformationen, die die Globalfunktion wesentlich beeinflussen, werden schon durch den Prozeß der Verschmelzung der letzten Nierenanlage, dem Metanephros, und der aus der Kloake aussprossenden Ureterknospe angelegt. Trifft dieser Verschmelzungsprozeß nicht orthograd aufeinander, kann es zu verschiedenen Formen der Dysplasie kommen, deren funktionelle Auswirkungen von der Position des Aufeinandertreffens der Ureterknospe mit dem Metanephros und dem dadurch entstandenen Ausmaß an Dysplasie abhängt. So kann man fehlabgehende Harnleiter aus dem Nierenbecken als Ureterabgabgansstenose erkennen. Dabei kann die Funktion der mehr oder weniger gestauten zugehörigen Niere völlig normal oder mehr oder weniger in ihrer Globalfunktion eingeschränkt sein. Über das Ausmaß der Funktionseinschränkung entscheidet die Stärke der Obsturktion, die schon mit Beginn der Nierenfunktion, d. h zwischen der 10 und 12. Schwangerschaftswoche aus das Nierenparenchym einwirk. Im Bereich der ableitenden Harnwege kann die Stauung durch eine Uretermündungsstenose oder meist beidseitig durch Obstruktion im Bereich der Urethra, bei den sog. Urethralklappen, ausgelöst sein.
