Summary
Fracture of the femoral shaft is a common lesion in children. Whereas conservative treatment has lost value during recent years, operative treatment is presently more favoured. ORIF with plating or closed reduction and external fixation are estimated to be no longer adequate in children. Prevot’s method of elastic intramedullary rodding seems to be a superior alternative. In a 3 year period, 15 children with a fracture of the femoral shaft were consecutively treated in our institution with closed reduction and intramedullary stabilization by titanium elastic nails (TEN). The youngest patient was 3-years-old, the oldest child 14 years.Very proximal and distal fractures were also included. In two cases of a near to hip-fracture, only an additional plaster cast was necessary. On average, all fractures healed without change of method within 7 weeks to full weight bearing. Titanium elastic nails proved to be a minimally-invasive and safe stabilization device for femoral shaft fractures in children.
Zusammenfassung
Die Femurschaftfraktur des Kindesalters stellt eine häufige Verletzung vor. Während die konservative Behandlung in den letzten Jahren erheblich an Bedeutung verloren hat, werden heute zunehmend operative Behandlungsformen favorisiert. Die offene Reposition mit der Plattenosteosynthese wie auch die geschlossene Einrichtung mit anschließendem Fixateur externe gelten inzwischen nicht mehr als kindgerecht. Als Alternative ist die elastische Markraumschienung nach Prevot anzuführen. In einem Dreijahreszeitraum wurden in unserer Institution 15 Kinder von 3 bis 14 Jahren mit einer Femurschaftfraktur mittels geschlossener Reposition und intramedullärer Stabilisierung durch elastische Titannägel versorgt (TEN). Eingeschlossen wurden auch die gelenknahen Schaftbrüche. Eine kurzzeitige zusätzliche Gipsbehandlung wurde nur bei 2 hüftnahen Brüchen angewandt.Alle Frakturen heilten im Durchschnitt innerhalb von 7 Wochen zur vollen Belastbarkeit aus.
