Summary
Methods: 20 Patients with ductal adenocarcinoma of the pancreatic head were examined pre- and postoperatively by oral glucose tolerance test (oGTT) according to WHO criteria. In three cases, a non-insulin dependent diabetes mellitus was known from clinical history. Results: In preoperative oGTT, only five patients had normal glucose tolerance (6 = impaired, 9 = diabetes mellitus) despite insulin secretion at a high level. Postoperatively, insulin secretion decreased significantly whereas glucose tolerance improved significantly (11 × normal glucose tolerance). Discussion: Preoperatively, there is probably peripheral insulin resistance caused by the tumor. Therefore, a diabetic metabolism can improve after duodenopancreatectomy. An impaired glucose tolerance can be an early sign of pancreatic cancer.
Zusammenfassung
Methodik: Bei 20 Patienten mit Adenokarzinom des Pankreaskopfes wurden prä- und postoperativ orale Glukosetoleranz-Tests (oGTT) nach WHO-Kriterien durchgeführt. In 3 Fällen bestand anamnestisch ein nicht-insulinpflichtiger Diabetes mellitus. Ergebnisse: Im präoperativen oGTT hatten nur 5 Patienten eine normale Glukosetoleranz 6 × eingeschränkt, 9 × Diabetes mellitus) trotz hoher Insulinsekretion. Postoperativ fiel die Insulinsekretion signifikant ab, die Glukosetoleranz besserte sich jedoch signifikant (11 × normale Glukosetoleranz). Diskussion: Präoperativ liegt vermutlich eine tumorbedingte periphere Insulinresistenz vor. Insofern kann sich eine diabetische Stoffwechsellage nach Pankreaskopfresektion verbessern. Eine eingeschränkte Glukosetoleranz kann ein frühes Zeichen des Pankreaskarzinoms sein.
