Summary

Treatment of intestinal ischemia needs close cooperation between general and visceral surgeon and radiologist. Angiography remains the gold standard, although vascular lesions are also perfectly visualized by CT-scan. In acute ischemia, angiography discriminates between occlusive disease which needs immediate laparotomy (embolectomy / vascular reconstruction and bowel resection) and nonocclusive disease. Angiography also leads to immediate treatment by selective infusion of vasodilative agents. In case of further diagnostic doubts, laparoscopy should be performed. Furthermore, mesenteric angioplasty builds a new sufficient alternative to endarterectomy and aortovisceral bypass in the treatment of chronic ischemia.

Zusammenfassung

Die Behandlung der mesenterialen Ischämie bedarf der engen Zusammenarbeit zwischen Visceralchirurgen, interventionell tätigen Gefäßchirurgen und Radiologen. Diagnostisches Standardverfahren für Gefäßläsionen bleibt, obwohl mittels Computertomographie gut darstellbar, die Angiographie. Sie unterscheidet bei akuter Ischämie zwischen Gefäßverschluß, der zur sofortigen Operation (Embolektomie / Gefäßrekonstruktion und Darmresektion) führt, und nicht-okklusivem Prozeß. Sie leitet zudem die Therapie durch selektive Vasodilatatien-Gabe ein. Die Laparoskopie kann diagnostische Unsicherheiten klären. Radiologisch-interventionelle Verfahren stellen in der Therapie chronischer Ischämien eine suffiziente Alternative zu Endarteriektomie und aortovisceralem Bypass dar.
