Zusammenfassung
Mangelernährung wird bei chronisch entzündlichen Darmerkrankungen (CED) häufig beobachtet: Etwa 2/3 aller Patienten mit Morbus Crohn und 1/3 aller Patienten mit Colitis ulcerosa weisen Zeichen der Unterernährung auf. Dies führt zu Gewichtsverlust und spezifischen Mangelerscheinungen wie Anämie, gestörten Immunfunktionen und Osteopenie. Bei Kindern kann es zu Minderwuchs kommen, der unbehandelt oft irreversibel ist. Deshalb sollten bei CED ernährungsmedizinische Maßnahmen einen Teil des Therapiekonzeptes darstellen. Es gibt außerdem Hinweise dafür, dass Ernährungsmaßnahmen, insbesondere die künstliche Ernährung, neben dem Ausgleich von Mangelsituationen auch primärtherapeutische Bedeutung haben könnten, wobei die zugrunde liegenden Mechanismen weitgehend unklar sind. Schließlich wird vermutet, dass Ernährungsgewohnheiten und einzelne Nahrungsmittel ätiologische bzw. pathogenetische Bedeutung für die Entstehung von CED bei Subgruppen haben könnten (Griffiths et al. 1995).
