Zusammenfassung
Das Spektrum der entzündlichen Erkrankungen von Dick- und Dünndarm ist bekanntermaßen breit. Es umfasst verschiedene ätiologisch definierbare chronisch-entzündliche Darmerkrankungen (CED) sowie zwei idiopathische Formen: Colitis ulcerosa und Morbus Crohn (Übersicht: von Herbay 1999). Die Gewissheit („certainty“) einer Diagnose Morbus Crohn und Colitis ulcerosa ist begrenzt. Gemäß der eigenen Erfahrung — bei mehr als 1.000 operationsbedürftigen CED-Patienten — bestätigt sich die präoperative Diagnose einer idiopathischen CED bei einem Viertel der Fälle histopathologisch nicht; dies betrifft vor allem diejenigen Patienten mit der Diagnose Morbus Crohn (von Herbay 1999). Zu den verkannten Diagnosen gehören u. a. auch infektiöse Darmerkrankungen mit chronischem Verlauf (infektiöse CED). Dieser Beitrag möchte aufzeigen, welche Möglichkeiten histopathologische Untersuchungen im Rahmen der Diagnostik infektiöser Darmerkrankungen bieten können und wo bzw. warum praktische Grenzen bestehen.
