Zusammenfassung
In der Therapie maligner Erkrankungen können toxische Chemotherapien oder Bestrahlungen und deren Kombinationen zu einer akuten Schädigung des Darmepithels mit Abflachung des Epithels und bisweilen bis zum Untergang kompletter intestinaler Krypten und Atypien der Kryptenzellkerne führen. Diese Veränderungen werden klinisch manifest durch krampfartige abdominelle Schmerzen, begleitet durch wässrige Diarrhöen und Gewichtsverlust. Die akute Schädigung ist in der Regel reversibel und es kommt zu einer Normalisierung der Epithelzellschicht nach etwa 16–18 Tagen. Bei anhaltender Granulozytopenie kann es dann zu einer Sonderform einer intestinalen Entzündung, der so genannten neutropenen Kolitis kommen.
