Pharmacokinetic Study of a Gentamicin/Clindamicin Bone Cement Used in One-stage Revision Arthroplasty

Abstract

Gram-positive organisms, primarily Staphylococcus aureus and epidermidis, continue to be the organisms most frequently responsible for infected joint prostheses. We investigated a new bone cement containing 1 g gentamicin and 1 g clindamycin per 40 g Palacos. The purpose of this prospective in vivo study was to measure the elution of gentamicin and clindamycin antibiotics from Palacos bone cement in one-stage hip revision arthroplasty. The study was performed on 24 patients who were suffering from infected total hip replacements. Venous blood samples, drainage fluid and urine were collected from each patient before and up to 10 days after operation. The average concentration in the drainage fluid was more than 100 times higher than in the serum. All infections were treated successfully. No recurrence or persistence of the periprosthetic infection was reported and no side-effects were observed during the first postoperative year.
