Zusammenfassung
Die Bedeutung humoraler Immunmechanismen bei der akuten Nieren-transplantatabstoßung stößt auf zunehmendes Interesse. Eine umfassende Diagnostik ist entscheidend für einen gezielten Einsatz »antihumoraler« Therapiestrategien (z.B. Immunadsorption). Während der letzten Jahre publizierte Studien favorisieren den immunhistochemischen Nachweis des Komplementspaltprodukts C4d als attraktiven Marker für die prognostisch ungünstige alloantikörpermediierte Abstoßungsreaktion. So sind kapilläre C4d-Ablagerungen im Nierentransplantat eng mit dem Nachweis spenderspezifischer Antikörper assoziiert. In einer retrospektiven Studie untersuchten wir bei biopsierten Nierentransplantatempfängern die Assoziation einer C4d-Positivität mit histologischen Kriterien einer Abstoßungsreaktion. Kapilläre C4d-Ablagerungen korrelierten nicht mit gängigen morphologischen Zeichen einer zellulären Abstoßung. C4d-positive Transplantatempfänger hatten, auch in Abwesenheit einer zellulären Abstoßung, eine schlechtere Transplantatfunktion als C4d-negative Patienten. Wir berichten über den erfolgreichen Einsatz einer Immunadsorptionsbehandlung bei einer Nierentransplantatempfängerin mit einer therapierefraktären C4d-positiven humoralen Abstoßungskrise. Unsere Beobachtungen unterstützen den früher postulierten diagnostischen Wert kapillärer C4d-Ablagerungen. Die tatsächliche Inzidenz und klinische Relevanz dieses Markers kann allerdings nur durch eine prospektive Studie geklärt werden.
