Zusammenfassung
In einer großen, prospektiven, randomisierten, multizentrischen Vergleichsstudie erhielten 557 nierentransplantierte Patienten Prograf® (Tacrolimus, n = 286) oder Neoral® (Ciclosporin-Mikroemulsion, n = 271), jeweils in Kombination mit Kortikosteroiden und Azathioprin. Das Patienten- und Transplantatüberleben war vergleichbar. Bei der Vermeidung akuter Abstoßungsreaktionen war die Tacrolimus-Therapie der Therapie mit Ciclosporin-Mikroemulsion (Ciclosporin-ME) überlegen. So lag die Inzidenz der bioptisch bestätigten akuten Abstoßungen unter Therapie mit Tacrolimus bei 19,6% gegenüber 37,3% unter Ciclosporin-ME (p<0,001). Auch kortikosteroidresistente und rekurrente akute Abstoßungen traten signifikant seltener unter Tacrolimus auf. Das Nebenwirkungsprofil der beiden Studiengruppen war im Wesentlichen vergleichbar, wobei sich die Tacrolimus-Therapie vorteilhaft hinsichtlich Hypertonie und Lipidstoffwechsel erwies.
