Zusammenfassung
Epidemiologische Daten belegen höhere Funktionsraten nach Lebendnierentransplantation selbst im Vergleich zu optimal gematchten Nieren von hirntoten Spendern. Als direkte Folge der schweren Hirnschädigung kommt es zu vermehrter Expression transplantationsrelevanter Gewebsantigene und vaskulärer Adhäsionsmoleküle in peripheren Organen. Im Transplantationsmodell werden Organe von hirntoten Tieren häufiger abgestoßen als von nichthirntoten narkotisierten Spendertieren. Klinische Daten deuten darauf hin, dass die Applikation von adrenergen Substanzen bei hirntoten Spendern vor der Organentnahme zu weniger akuten Abstoßungsepisoden nach Nierentransplantation führt und auch die Transplantatfunktionsrate im Langzeitverlauf verbessert. Nach 4 Jahren ist der günstige Effekt quantitativ vergleichbar mit prospektivem HLA-Matching auf Klasse-I- oder -II-Antigenen. Diese Effekte sind durch hämodynamische Wirkungen der Katecholamine allein nicht zu erklären. In Zellkultur hemmen adrenerge Substanzen bei klinisch relevanten Konzentrationen die durch TNF-α oder IL-lß stimulierte Expression von Adhäsionsmolekülen (E-Selektin, VCAM) auf Endothelzellen. Zudem vermag Dopamin das protektive Enzym Hämoxygenase-1 zu induzieren, das das Transplantat vor oxidativem Stress schützen könnte. Eine optimierte organprotektive Therapie, die bereits vor der Explantation einsetzt und die selektive Gabe von Katecholaminen einschließt, könnte die Langzeitergebnisse nach Nierentransplantation ohne den Preis von Nebenwirkungen für den Transplantatempfänger maßgeblich verbessern.
