Zusammenfassung

Seit der ersten Anwendung von Curare vor über 50 Jahren ist die anästhesiologische Forschung und fachspezifische Industrie bemüht, das sog. „ideale Muskelrelaxans“ zu finden, dessen wichtigste Eigenschaften neben der nichtdepolarisierenden Wirkungsart v. a. rasche Anschlagzeit, kurze Wirkdauer, schnelle Erholung sowie Fehlen von Nebenwirkungen und aktiven Metaboliten sein sollten. Einige dieser Ziele konnten bereits mit der Einführung der derzeit am häufigsten verwendeten Präparate Atracurium und Vecuronium erreicht werden. Eine der wichtigsten Eigenschaften jedoch, die kurze Anschlagzeit, die eine frühzeitige Intubation ermöglicht, erzielt man bis jetzt nur durch die Anwendung des depolarisierend wirkenden Succinylcholin. Dessen Verabreichung ist jedoch mit einer großen Anzahl von unerwünschten Nebenwirkungen behaftet. In der Vergangenheit wurden einige Versuche unternommen, die Anschlagzeit nichtdepolarisierender Substanzen zu verkürzen. Aber weder die Verabreichung einer höheren Intubationsdosis (4- bis 6-fache ED90) [4] noch die Anwendung des sog. „priming principle“ [14] konnten Succinylcholin zur raschen Intubation ersetzen.
