Zusammenfassung
Polymere Werkstoffe haben sich einen festen Platz in der orthopädischen Chirurgie erobert. Sie kommen vor allem beim Gelenkersatz und als Fixationsmittel von Gelenkprothesen (sog. Knochenzement) zur Anwendung, in geringerem Umfang auch als Schrauben und Pins zur Knochenrekonstruktion, als Bandersatz und in bestimmten Anwendungen auch als Trägersubstanz für Arzneimittel (z.B. Wirkstoffträger für Antibiotika). Im Hinblick auf ihre Herstellung lassen sich bei den eingesetzten Werkstoffen dabei synthetische und teilsynthetische Polymere unterscheiden. Teilsynthetische Polymere kommen hauptsächlich bei resorbierbaren Implantaten zum Einsatz (Polyglykole, Polylactide). Aufgrund ihrer geringen mechanischen Stabilität, des starken Festigkeitsabfalls bei der Resorption und der beim Abbau in großen Mengen entstehenden sauren Valenzen können solche resorbierbaren polymeren Implantatwerkstoffe bisher nur für Formkörper mit geringen Abmessungen genutzt und an Stellen implantiert werden, an denen die zu übertragenden Kräfte gering sind. Beispiele dafür sind Pins zur Fixation von Brüchen der Fußwurzelknochen oder von Mittelfußknochen bei Korrekturoperationen, chirurgisches Nahtmaterial oder kleine Schrauben. Der Ersatz zerstörter Gelenkflächen, die Gelenkendoprothetik, ist dabei unter allen genannten zum bedeutendsten Einsatzgebiet für Polymere in der orthopädischen Chirurgie geworden, weshalb sich der nachfolgende Beitrag vor allem mit dieser Problematik beschäftigt.
