Zusammenfassung
Die Behandlung chondraler und osteochondraler Gelenkdefekte stellt den behandelnden Arzt häufig vor die Aufgabe, aus einer Vielzahl von therapeutischen Optionen das geeignete Verfahren auswählen zu müssen. Zur Anwendung kommen Refixationen von osteochondralen Fragmenten mittels resorbierbarer Stifte und Schrauben, das Debridement mit Shaver und Lasersystemen und die sogenannten marrow stimulations (Abb. 1), unter denen Verfahren subsummiert werden, bei denen es durch Eröffnung der subchondralen Knochenlamelle zur Bildung eines „fibrin clot“ kommt, der über pluripotente Stammzellen die Bildung eines fibrokartilaginären Regeneratknorpels induzieren kann. Das mechanisch minderwertige Regeneratgewebe bedingt jedoch nur über einen kurzen Zeitraum gute klinische Ergebnisse [6].
