Zusammenfassung
Die Tracheostomie zählt zu den ältesten chirurgischen Eingriffen und wurde bereits vor über 2000 Jahren durchgeführt [5]. Während die Tracheostomie früher eher als Notfalleingriff bei traumatisch oder entzündlich bedingten Verlegung der oberen Atemwege als ultima ratio in Betracht kam, gilt sie seit Anfang dieses Jahrhunderts als elektiver Eingriff [22]. Mit der Entwicklung der Intensivmedizin in den 50er und 60er Jahren des 20. Jahrhunderts ergab sich für die Tracheostomie ein neues Anwendungsgebiet im Rahmen der zunehmend durchgeführten Langzeitbeatmung [15]. Seitdem konkurrieren Luftröhrenschnitt und endotracheale Intubation als Beatmungsmodalität des Intensivpatienten, und vielfach wird die Indikation zum einen oder anderen Verfahren intuitiv und individuell gestellt. Die indikatorischen Unsicherheiten werden auch durch die Entscheidungen der amerikanischen Konsensus-Konferenz aus dem Jahr 1989 deutlich [28]. Hier empfahlen die Experten bei einer Beatmungszeit bis zehn Tagen eine Intubation, ab einer Beatmungsdauer über 21 Tage eine Tracheostomie. Die Zwischenphase, die für den Großteil der beatmeten Intensivpatienten zutrifft, ist nach wie vor unklar und läßt Freiraum für individuelle Entscheidungen.
