Abstract
The major long term failure mode of total knee arthroplasty is polyethylene wear and its sequellae. These sequellae can include osteolysis, implant loosening, and instability. In an attempt to minimize such polyethylene wear surgeons and implant designers have attempted several modifications of the “traditional approach” to knee replacement. These modifications have included the use of more conforming articular surfaces so as to decrease contact stresses and modifying the properties of the polyethylene itself, and even considering using hard on hard bearing surfaces to completely eliminate the polyethylene. This paper will describe the approach that our group has taken in attempting to decrease polyethylene wear, namely modifying the surface articulating against the polyethylene.
