Zusammenfassung
Bereits seit über 40 Jahren beschäftigen sich zahlreiche Autoren mit der Entwicklung von künstlichen Bandscheiben, so soll bereits Präsident J.F. Kennedy Ende der 50er Jahre eine der ersten Prothesen vom Fernström-Typ erhalten haben [17]. Zahlreiche Entwicklungen jedoch blieben im Laufe der Zeit nur theoretisch und wurden bis heute nicht klinisch eingesetzt — allein im US-Patentamt finden sich über 80 verschiedene Konstruktionen zum Bandscheibenersatz. Die Pathophysiologie und Biomechanik der Bandscheibendegeneration und ihre vorzeitige Entstehung durch eine Nukleotomie im Segment oder Fusion im Nachbarsegment, der so genannten Anschlussdegeneration, werden ebenfalls schon seit Jahren untersucht, allerdings konnten viele Fragen bis heute nicht abschließend beantwortet werden. Die wichtigste Frage nach der Ursache und dem Ort der eigentlichen Schmerzentstehung ist immer noch Gegenstand kontroverser Diskussionen, ebenso wie der Begriff der Instabilität. Nach dieser langen Zeit der Forschung und Entwicklung, bei anfänglich nur mäßigen Erfolgen im Vergleich zur übrigen Prothetik und später auch Fusion, erfreut sich der Bandscheibenersatz nun erneut einer wachsenden breiten Aufmerksamkeit. Dies ist überwiegend bedingt durch erste ermutigende klinische Ergebnisse, verbesserte Werkstoffe und die zunehmende Anzahl an Patienten, die operativ behandelt werden müssen und eine Behandlungsalternative zur Fusion wünschen. Eine vorläufige Klassifikation der derzeitigen technischen und biologischen Möglichkeiten wurde von Maryanchik [17] unternommen, der eine Einteilung in sieben Klassen vorschlägt.
