Zusammenfassung
Das bullöse Pemphigoid (BP) ist mit Autoantikörpern (AAk) gegen zwei hemidesmosomale Strukturproteine der Haut, BP180 und BP230, assoziiert. Der Antikörpertiter in der indirekten Immunfluoreszenz (IF) korreliert jedoch nicht mit der Krankheitsaktivität der Patienten. In der vorliegenden Studie gingen wir der Frage nach, ob die Serumspiegel der anti-BP180 AAk die Krankheitsaktivität besser widerspiegeln als die Titer der indirekten IF. Wir untersuchten die Seren von 15 BP konsekutive Patienten jeweils vor Therapie und 4 und 8 Wochen später mittels indirekter IF auf NaCl-separierter humaner Spalthaut und im ELISA mit der rekombinanten BP180 NC16A Domäne als Zielantigen. Zur selben Zeit wurde die Krankheitsaktivität anhand der Anzahl der Läsionen beurteilt. Neun Patienten wurden mit Dapson plus Methylprednisolon (Da/M Gruppe) und 6 Patienten mit Doxycyclin plus Nicotinamid (Do/N Gruppe) behandelt. In beiden Therapiegruppen korrelierte die Krankheitsaktivität mit der BP180-Reaktivität im ELISA (p = 0,0072 bzw. 0,0040), nicht jedoch mit den Titern der indirekten IF (p = 0,16 bzw. 0,18). In der Da/M Gruppe korrelierten zudem Krankheitsaktivität und benötigte Steroiddosis (p = 0,0022). In dieser Studie zeigen wir erstmals, daß anti-BP180 AAk in direktem Zusammenhang mit der Krankheitsaktivität der Patienten stehen. Unsere Ergebnisse unterstreichen die pathogenetische Bedeutung von anti-BP180 AAk und zeigen, daß der BP180 ELISA zur Therapiekontrolle des BP geeignet ist.
