Zusammenfassung
Die elektive Lymphknotendissektion (ELND) ist seit fast 20 Jahren Gegenstand kontroverser Diskussionen. Durch die Einführung der Schildwächter-Lymphknoten-Exstirpation (SLKE) besteht jedoch jetzt die Möglichkeit, Patienten, die aufgrund klinisch und sonographisch okkulter Mikrometastasen von einer ELND profitieren könnten, zu identifizieren. Seit Oktober 1997 führten wir bei 140 Patienten mit melanozytären und nichtmelanozytären Tumoren der Haut die SLKE nach lymphoszintigraphischer und zusätzlicher Farbstoffmarkierung durch. Bei positivem SLK wurde in zweiter Sitzung eine radikale Lymphknotendissektion der betroffenen Region angestrebt. Von 119 auswertbaren Patienten mit malignem Melanom war in 87% ein unilateraler Lymphabstrom nachweisbar. Im Gesamtkollektiv betrug die Rate positiver Befunde in Lymphknoten 9,2% (11 Patienten). Bei Patienten mit einer Tumordicke > 1,5 mm (Stadium II) stieg die Rate auf 17,3%. Nach einer medianen Nachbeobachtungszeit von 8 Monaten ist bei einem Patienten mit positivem SLK eine weitere Lymphknotenmetastase aufgetreten. Die SLKE stellt im Gegensatz zur radikalen Lymphknotendissektion eine vergleichsweise einfache, komplikationsarme und kostengünstige Diagnostikmethode dar, die ein präziseres Staging der Patienten erlaubt. Mit der SLKE können Subkollektive identifiziert werden, die von einer ELND und/oder einer adjuvanten Chemo-und/oder Immuntherapie profitieren könnten.
