Zusammenfassung

Ein 58 jähriger Transportarbeiter wird nach Beginn der Frühschicht um 6:20 Uhr mit heftigen links-thorakalen Schmerzen als Notfall eingeliefert. Im EKG werden ST-Strecken-Hebungen in den Abteilungen II, III, aVF festgestellt, und eineinhalb Stunden nach Schmerzbeginn wird eine thrombolytische Therapie durchgeführt. Der Patient hat nie geraucht, war übergewichtig (92 kg, 179 cm, Body-Mass-Index 28,7 kg/m2), hatte erhöhte Triglyzeridwerte (270 mg/dl), ein geringfügig erhöhtes Cholesterin (245 mg/dl) und am Aufnahmetag eine Blutzuckerkonzentration von 260 mg/dl. Der Vater des Patienten litt an einem Typ-2-Diabetes und verstarb 72 jährig an einem Myokardinfarkt. Die Untersuchungen ergaben einen HbAic-Wert von 11,0%, Urinuntersuchungen auf Ketonkörper ergaben negative Befunde oder allenfalls Spuren.
