Zusammenfassung
Die gebräuchlichste Methode zur Achskorrektur an der unteren Extremität ist die Keilentnahme in der Technik nach Coventry [1]. Die Osteotomie erfordert jedoch einen offenen Zugang zum Knochen und belastet die umgebenden Weichteile entsprechend. In Kombination mit der beschriebenen partiellen Resektion des Fibulaköpfchens resultiert hieraus eine potentielle Gefährdung des N. peronaeus. Des weiteren entsteht durch die Keilentnahme (closing wedge) eine oftmals unerwünschte interligamentäre Verkürzung mit relativer Kollateralbandinstabilität. Die Umstellung des Knochens durch einen sich öffnenden Keil (opening wedge) erfordert die Interposition eines Autograft oder einen 180°-gedrehten Halbkeil vom Ort der Umstellung. Perkutane Osteotomietechniken sowie die Verwendung von Fixateursystemen zur Achseinstellung und Osteosynthese reduzieren das Weichteiltrauma erheblich.
