Abstract
The spleen is a lymphoreticular organ that is schematically arranged as a two-unit system consisting of the white pulp surrounded by areas of red pulp. The white pulp acts as a lymph node, whereas the red pulp is the site of the reticuloendothelial system, acting as a filtration system to remove normal and abnormal blood cells, microorganisms and particles. Splenic function is complex, and a variety of diseases, mainly affecting other organ systems, can interfere with splenic function and structure. For didactic reasons and because of the uncertain etiology of many of these disease entities, systemic diseases with splenic involvement will be classified into three different groups, depending on their site of action in the two-unit system of the spleen.
