Die Bedeutung der Erfahrung für die Ergebnisse in der Rektumchirurgie anhand einer prospektiven Multizenterstudie / The Role of Caseload Experience for the Short-Term Results Achieved in Rectal Surgery: Results of a Prospective Multicentre Study

Summary

The investigation was designed as a multicentre study involving 75 German hospitals that was carried out between January 1, 1999, and December 31, 1999. A total of 3756 patients were admitted to the study, of whom 1463 had a rectal carcinoma. The hospitals were divided into groups by caseload in the period under investigation as follows: <20, 20–40 and >40. The groups were identical in terms of age, sex, height weight, tumour stage, risk factors, and ASA classification. For a hospital volume of more than 40 patients/year the rectal excision rate, and for a hospital volume of more than 20 patients/year, the postoperative morbidity were significantly, reduced. The importance of a large caseload for the results of rectal surgery is reflected in a significant reduction in rectal excision rates and postoperative morbidity.

Zusammenfassung

Die Untersuchung wurde in Form einer prospektiven Multizenterstudie an 75 ostdeutschen Kliniken vom 1.1.–31.12.1999 durchgeführt. Es wurden 3756 Patienten er-faßt, davon 1463 mit einem Rektumkarzinom. Es erfolgte eine Unterteilung der Kliniken in solche mit einer Fallzahl von <20, 20–40 und >40 behandelten Patienten mit einem Rektumkarzinom. Die Gruppen waren identisch in Hinblick auf Alter, Geschlecht, Größe, Gewicht, Tumorstadien, Risikofaktoren, ASA-Klassifikation. Die Rektumexstirpationsrate war bei einer Fallzahl über 40 Patienten/Jahr und die postoperative Morbidität bei einer Fallzahl von über 20 Patienten/Jahr signifikant vermindert. Der Wert einer großen Fallzahl für die Ergebnisse in der Rektumchirurgie liegt in einer signifikanten Reduktion der Rektumexstirpationsraten und in der Verminderung der postoperativen Morbidität.
