Laparoskopischer Wiederanschluß nach Hartmann-Operation / Laparoscopicaly Guided Reversal of Hartmann’s Procedure

Summary

In 23 patients laparoscopically guided reversal of Hartmann’s procedure was attempted. The postoperative course was followed prospectively. Results: In three cases (14%) conversion to the conventional technique was necessary due to adhesions. Median operative time was 114 (65-180) min. One patient developed an anastomotic breakdown. The stoma was reestablished. Three patients showed a wound infection. Their convalescence was fast (first evacuation 3.3 days, complete oral nutrition 3.6 days, hospitalisation 7.5 days). One patient developed an anastomotic stricture which needed endoscopic dilatation. Conclusion: Laparoscopically assisted Hartmann’s reversal is technically demanding but feasible. Postoperative morbidity is low.

Zusammenfassung

Bei 23 Pat. mit vorheriger Hartmann-Resektion wurde ein lap. ass. Wiederanschluß angestrebt. Der postop. Verlauf wurde prospektiv erfaßt. Ergebnis: Bei 3 Pat. (14%) wurde aufgrund ausgeprägter Adhäsionen konvertiert. Die Op-Dauer betrug im Mittel 114 (65–180) Min. Ein Pat. entwickelte eine Anastomoseninsuffizienz. Erneut mußte ein Stoma angelegt werden. Eine Wundheilungsstörung trat bei 3 Pat. auf. Die Rekonvaleszenz war rasch (Darmtätigkeit nach 3,3 Tagen, volle Oralisierung 3,6 Tage, KH-Verweildauer 8,5 Tage). Ein Pat. entwickelte nach 6 Monaten eine Anastomosenstenose, die endoskopisch erfolgreich dilatiert wurde. Schlußfolgerung: Der lap. ass. Wiederanschluß nach Hartmann-Op ist technisch anspruchsvoll, aber möglich. Er weist eine geringe Morbidität auf.
