Magenfrühkarzinom: Endoskopische Therapie / Early Gastric Cancer: Endoscopic Therapy

Summary

Endoscopie removal of early gastric cancers is limited to low risk situation: T_1m (=mucosa) L₀ V₀ G_1–2 R₀. In 40 patients pretherapeutic staged as low risk cancers the complete endoscopie removal of the lesion was performed. In 75% (29 out of 40) the pathological specimen proved a low risk cancer. In 16% (5 out of 29) we found recurrency of the tumor (follow up median 2.5 years). These neoplasms were also removed. From the 25% highrisk cases (S_m, G₃) 11 patients were operated on. Only 2 patients had residual tumor. The complication rate of the endoscopie procedures was only 10% (4x bleeding with transfusion of blood-units, l × perforation), no patient died.

Zusammenfassung

Die Basis für die Entscheidung zu einer lokalen endoluminalen Therapie eines Magenfrühkarzinoms stellt das histologische Staging dar. Deshalb wird die Läsion bei vermuteter Low-risk-Situation zunächst endoskopisch komplett entfernt. Ergibt sich ein T₁ L₀ V₀ G_1-2 R₀-Stadium ist hiermit die definitive kurative Therapie erfolgt. Nur das Stadium T_1m L₀ V₀ G_1-2 R₀ ist zugelassen für die endoskopische Behandlung. Bei 40 Patienten (Altersdurchschnitt 75 Jahre) mit Magenfrühkarzinom fanden sich in 75% (29 von 40) eine Low-risk-Situation. Bei 16% (5 von 29) traten im Follow up nach durchschnittlich 2½ Jahren Rezidive auf, welche erneut endoskopisch entfernt wurden. Von den High-risk-Fällen (25% = 11 von 40) wurden 9 operiert. Bei nur 2 Fällen fand sich ein Residualtumor. Die Komplikationsrate der endoskopischen Verfahren lag bei 10% (4x transfusionspflichtige Blutung, l × Perforation), die Letalität bei 0%.
