Zusammenfassung
Die chronischen Virushepatitiden (Nomenklatur in Tabelle 1) stellen für den Pädiater diagnostisch und therapeutisch durchaus eine Herausforderung dar. Der häufigste Übertragungsweg für die chronischen Virushepatitiden (seit der Einführung des Blutproduktescreenings) ist die perinatale Transmission. Bei Kindern, die auf diesem Weg infiziert wurden, fehlen lange Zeit klinische Zeichen der Infektion. Laborchemisch ist oft keine oder nur eine milde Transaminasenerhöhung nachweisbar. Obwohl die Progression zur Leberzirrhose oder dem hepatozellulären Karzinom nur in seltenen Fällen schon in das Kindes- und Jugendalter fällt, muss davon ausgegangen werden, dass langfristig weltweit etwa 30% der Träger an den Folgen der Infektion versterben. Die schlechte Prognose legt eine therapeutische Intervention nahe, die sich bisher jedoch nur auf die Behandlung mit α-Interferon beschränkte. Die Erfolgsrate war jedoch eher enttäuschend, da nur etwa bei 25–40% der Patienten das Therapieziel erreicht wird. Die Behandlung hat zudem eine Reihe von unerwünschten Nebenwirkungen.
