Zusammenfassung
Die Wirkung eines Arzneimittels hängt ab von seiner Konzentration am Wirkungsort. Daher wäre für eine optimale Einstellung eines depressiven Patienten auf ein Antidepressivum die Messung der Hirnkonzentrationen die beste Methode. In der klinischen Routine ist jedoch die Messung der Konzentration eines Antidepressivums oder einer anderen psychoaktiven Substanz im Gehirn von Patienten nicht durchführbar. Solche Untersuchungen erfordern aufwendige Verfahren, wie die Positronenemissionstomographie. In der Praxis steht die Messung der Antidepressiva-Konzentrationen im Blut, der sogenannten Blutspiegel (analog auch Plasma- oder Serumspiegel) als Surrogat-Parameter für Hirnkonzentrationen, zur Verfügung. Tierexperimentelle Untersuchungen haben für verschiedene Antidepressiva gezeigt, daß die Blutspiegel wesentlich besser mit den Konzentrationen im Gehirn korrelieren als die Dosis (Glotzbach u. Preskorn 1982; De Vane et al. 1999). Die nach Einnahme einer gleichen Dosis eines Antidepressivum resultierenden Wirkstoffkonzentrationen sind im Blut und sicherlich auch im Gehirn interindividuell hoch variabel. Dies trifft auf alle Antidepressiva zu. Individuelle Unterschiede in der Compliance und in der Ausstattung der Leber mit Fremdstoff metabolisierenden Enzymen, für Antidepressiva insbesondere aus der Gruppe der Cytochrom P450 (CYP)-Familie, bestimmen wesentlich die Variabilität der Blutspiegel. Daher ist es schwierig, allein durch die Dosis bei jedem Patienten einen optimal wirksamen Blutspiegel zu erzeugen. Bei etwa 50% der Patienten muß daher die Dosis korrigiert werden.
