Zusammenfassung
In den letzten Jahren wurde durch mehrere bevölkerungsbezogene epidemiologische Studien sowie Fall-Kontroll-Studien eine Reihe von Komorbiditäten zwischen der Migräne und anderen Erkrankungen wahrscheinlich gemacht. Am häufigsten konnte dieses für psychiatrische Erkrankungen wie Depression, Angsterkrankungen und soziale Phobien bestätigt werden, sichere Zusammenhänge finden sich aber auch für Migräne und Schlaganfall, verschiedene Schwindelformen und Fibromyalgiesyndrom. Parallel dazu konnten einige wesentliche pathophysiologische Mechanismen der Migräne gefunden werden, einerseits wird eine passagere Störung von Ionenkanälen als eine Ursache der Migräne angenommen, andererseits finden sich Hinweise, die für eine primäre Reizverarbeitungsstörung und nachfolgende kortikale Funktionsstörung mit „spreading depression“ als primäre Ursache sprechen. Gemeinsame Endstrecke ist eine Aktivierung des trigemino-vaskulären Systems mit Induktion einer aseptischen Entzündung in den Gefäßwänden der großen duralen Blutleitern.
