Opioide werden in reine Agonisten (Morphin, Fentanyl, Alfentanil, Sulfentanil, Remifentanil, Pethidin, Piritramid, Tramadol, Oxycodon und Hydromorphon), Agonisten-Antagonisten (Buprenorphin, Nalbuphin), Partialagonisten (Buprenorphin, L-Methadon) und reine Antagonisten (Naloxon) unterteilt. Entsprechend dieser Klassifizierung stellt das Kapitel die wichtigsten Pharmaka hinsichtlich Wirkungsweise, Dosierung, Nebenwirkungsprofil und Kontraindikationen vor.
