Zusammenfassung

Hämostase bezeichnet diejenigen physiologischen Prozesse, die bei struktureller Schädigung des Gefäßsystems die Blutstillung gewährleisten, den Blutverlust begrenzen und dadurch die Aufrechterhaltung eines kontinuierlichen Blutflusses zur Sicherstellung der nutritiven Versorgung des Gewebes ermöglichen.

Weit über die einfache Begrifflichkeit hinaus hat sich unser Verständnis hinsichtlich der Abläufe im Hämostasesystem kontinuierlich weiterentwickelt. Das von MacFarlane (1964) bzw. Davie u. Ratnoff (1964) aufgestellte Konzept der zwei Kaskaden begreift den Gerinnungsprozess als Abfolge proteolytischer Reaktionen und stellt einen ersten Versuch der Systematik dar. Das Postulat einer extrinsischen und intrinsischen Aktivierung der plasmatischen Gerinnung gilt jedoch zwischenzeitlich als überholt, wissen wir doch heute aus zahlreichen Untersuchungen, dass unter physiologischen Bedingungen die Initiation der plasmatischen Gerinnung ausschließlich auf dem extrinsischen Wege über Gewebsthromboplastin erfolgt. Darüber hinaus vermag das Kaskadenmodell den Beitrag der zellulären Komponente an der Clot-Bildung noch nicht in adäquater Weise zu integrieren und ist daher nicht geeignet, die Abläufe in vivo zu beschreiben.
