Zusammenfassung
Kalziumantagonisten sind seit einigen Jahren in der Behandlung der koronaren Herzkrankheit und der arteriellen Hypertonie etabliert. Ihre therapeutische Wirkung wurde zunächst hauptsächlich auf ihren Einfluß auf die Hämodynamik zurückgeführt. Pharmakologische Befunde deuten jedoch auf eine direkte Beeinflussung der Atherogenese hin. Die Behandlung von cholesteringefütterten Kaninchen mit Dosen von Nifedipin, die kontinuierlich ausreichende Plasmaspiegel sicherstellten, verminderte den Anstieg des Gefäßwandcholesterins und -kalziums sowie die Plaquebildung in den Aorten um etwa die Hälfte (3, 4, 17, 18). Ein ähnlicher antiatherogener Effekt von Nifedipin war an cholesteringefütterten Rhesusaffen zu beobachten (7). Bei normolipämischen Ratten und Kaninchen wurde die Ballonkatheter-bedingte myointimale Proliferation des Carotisendothels durch 2 × tgl. 1, 10 oder 50 mg/kg Nifedipin um 2%, 50% und 62% gehemmt. Dabei waren andere proliferierende Gewebe wie Knochenmark, Testes oder Duodenum nicht gehemmt (5). Ähnlich wirksam waren Nifedipin und Nitrendipin gegenüber der kälteinduzierten Bildung atherosklerotischer Plaques in den Karotiden von lipidreich gefütterten Ratten (16).
