Zusammenfassung

Unter den klinischen Manifestationen der koronaren Herzerkrankung ist der akute Myokardinfarkt für Patient und Arzt gleichermaßen die dramatischste Erscheinungsform. Wie der plötzliche Herztod, die symptomatische und asymptomatische (stumme) Myokardischämie gehört er zu den ischämischen Syndromen. Die Herzinsuffizienz nach Myokardinfarkt ist dagegen ein Folgezustand nach abgelaufener transmuraler Ischämie. Rhythmusstörungen können sowohl während akuter ischämischer Ereignisse als auch nach abgelaufenem Infarkt auftreten und haben ihren Ursprung meist im arrhythmogenen Substrat, d.h. den noch nicht vernarbten, häufig noch ischämischen Randzonen von Infarkten. Wenn es sich dabei um anhaltende Kammertachykardien oder um Kammerflimmern handelt, sind sie die Ursache des plötzlichen Herztodes.
