Zusammenfassung
Durch die Einführung der thrombolytischen Therapie zur Minimierung der Myokardschädigung beim akuten Myokardinfarkt (AMI), welche ihre Stellenwert bewiesen haben, steht an erster Steile die schnelle Infarktdiagnose. Die thrombolytische Therapie ist nur dann erfolgreich, wenn sie innerhalb von 6 Stunden nach Entstehen der ersten Symptome angewendet wird [14,16]. Außerdem wird eine adäquate Diagnostik unter Berücksichtigung der Nebeneffekte und des hohen Selbstkostenpreises vorausgesetzt. Während der ersten Stunden des Infarktes ermöglichen die klinischen Symptome und die elektrokardiographischen (EKG) Untersuchungen nicht immer eine richtige Diagnose. Deshalb gibt es einen Mangel an frühzeitigen biochemischen Markern für die Diagnostik des Herzinfarktes. Da die Diffusionsgeschwindigkeit der intrazellulären Molekule von ihrem Molekulgewicht abhängig ist [15], kann eine frühzeitige Diagnose aus theoretischen Gründen nur mittels Bestimmungen von Kleinmolekülen verwirklicht werden. Deshalb gab es eine besondere Aufmerksamkeit für Markersubstanzen, wie z.B. Myoglobin (17 kD) [13,22] und Troponin T (37 kD)[18]. Dennoch ist die Analyse der Kleinproteine mittels Immunoassay zeitaufwendig und deshalb für ein Notfalllabor weniger geeignet. Außerdem wurde die diagnostische Spezifizität der Myoglobinbestimmung in Frage gestellt. Der menschliche Herzmuskel enthält große Mengen an Creatin (etwa 2–3 mg/g Herzgewebe) [33]. Im Gegensatz zu Herzenzymen (Molekulargewicht um 100 kD) und anderen Muskelproteinen (z.B. Myoglobin) ist Creatin wegen seines sehr niedrigen Molekulargewichtes (31) für die Frühdiagnostik des Infarktes sehr geeignet. Obwohl Creatinbestimmungen schon längst möglich sind, war die Bestimmung im Urin mittels üblicher kolorimetrischer Methoden wenig sensitiv. Erst seit Mitte der 80er Jahre steht die gentechnisch hergestellte Creatinase (Creatinamidinohydrolase, EC 3.5.3.3) in wirtschaftlichen Mengen für diagnostische Zwecke zur Verfügung. Diese Voraussetzungen bilden die Basis der Entwicklung der enzymatischen Creatinbestimmung als ein frühzeitiger Marker für die Herzinfarkt-Diagnostik.
