Erfahrungen und Umgang mit Mapping und Ablation in epikardialen Strukturen

Zusammenfassung

Das Mapping im Koronarsinus und in der V. cordis magna ist seit über 10 Jahren etablierter Bestandteil der interventioneilen ablativen Therapie des WPW-Syndroms, es dient hier zur Lokalisationsdiagnostik und Orientierungshilfe bei der Hochfrequenz(HF)ablation linksseitiger akzessorischer Leitungsbahnen, die entweder retrograd transaortal, ggf. transmitral oder antegrad transseptal erfolgt. Nur im Ausnahmefall wird das epikardiale Mapping in den verschiedenen Ästen des Koronarvenensystems zur Lokalisationsdiagnostik ventrikulärer Tachykardien (VT) oder gar zur Ablation selbst benutzt, ebenso selten wird im epikardialen Koronararteriensystem ein Mapping bzw. eine Ablation z.B. mittels Äthanol durchgeführt. Über Erfahrungen mit dem nichtchirurgischen transthorakalen epikardialen bzw. intraperikardialen Mapping bei ventrikulären Tachykardien liegen erste Berichte vor. In der eigenen Arbeitsgruppe konnten erste Erfahrungen mit multipolarem Mikrokathetermapping sowie ablativen Prozeduren in Koronarvenen und -arterien gesammelt werden. Die vorliegende Arbeit soll die bisher verfügbaren Daten zum epikardialen Mapping insbesondere bei ventrikulären Tachykardien vorstellen und diskutieren.
