Sklerafixation — new technique with adapted materials

Zusammenfassung

Bei der transkleralen Fixation von Hinterkammerlinsen hängt die Länge der Operation und der Schwierigkeitsgrad der Ausführung zu einem großen Teil ab von den angewandten Materialien, insbesondere vom Nahtmaterial und von der Art der Nadel.

In diesem Video wollen wir einen neuen Prolene-Schlaufenfaden vorstellen, welcher zeitaufwendige Verknotung der Naht an der Linsenhaptik überflüssig macht. Dieser ProleneSchlaufenfaden kann sowohl für die Innenstichtechnik als auch für die Außenstichtechnik verwendet werden. Das Video zeigt die Verwendung des Prolene-Schlaufenfadens im Rahmen einer neuen Außenstichtechnik, wobei die transklerale Vernähung durchgeführt wird, indem die Sklera mit einer Kanüle an der Basis von Sklerataschen in der 3.00- und 9.00-h-Position mit Kanülen durchstochen wird. Die Kanüle wird sodann mit der Nadel des Schlaufenfadens vom Augeninneren her gefüttert und die Kanüle wird atraumatisch zurückgezogen. Gegenwärtig haben wir mit dieser modifizierten Außenstichtechnik 60 Patienten operiert. Unsere klinischen Ergebnisse sind vielversprechend, da die Komplikationen, die der transkleralen Fixation von Hinterkammerlinsen angelastet werden können mit dieser Technik erheblich verringert werden konnten.

In the case of transcleral fixation of posterior chamber lenses the length the of the operation and the difficulties encountered in carrying out are to a large extent dependant on the material used — especially the suture material — and the type of needle.

In this video we would like to introduce a prolene loop suture, which eliminates time consumming knotting of the suture on the haptic of the lens. This prolene loop suture can be used for ab interno and ab externo technique. The video shows the use of the prolene loop suture for a new ab externo technique, where the transcleral suturing is externally performed by piercing the sclera with a cannula on the base of scleral pocket incisions in the 3 and 9 o’clock positons. The needle of the loop suture is inserted into the cannula, which in then gently retracted. At the present time 60 patients have been operated on with this modified ab externo-technique. The clinical results are encouraging, as the rate of complications related to transcleral fixation of posterior chamber IOLs could be considerably reduced.
